Scott Air Force Base Medical Center Retirees are encouraged to join TRICARE Prime, since at this time (Oct 2000) the base medical facilities have room to enroll more retirees in the program using Scott as their primary provider. St. Louis Air National Guard Retiree Activities Office The 131st Air National Guard Fighter Wing located at St. Louis (Lambert) International Airport also has a Retiree Activities Office. Their office is open limited hours, so we recommend that you call first, if you are requesting their assistance. Their telephone numbers are 314-263-6327 or DSN 693-6327. They can help with arrangements for ID cards, vehicle registration renewals and similar concerns.
In the event of the death of an Air Force retiree The Air Force Casualty Assistance office (a part of the Air Force Personnel Center at Randolph AFB, TX) should be notified in the event of an Air Force retiree's death. Their toll-free phone number is 1-877-353-6807. Alternate numbers are (210) 565-7849 (during duty hours) or (210) 565-3505 (after duty hours). These last two numbers are not toll-free. You should enclose a copy of the death certificate, the members's full name, Social Security Number, rank and date of retirement. If you are notifying them by phone, please be prepared to give the date, place (city and state), and cause (laymen's terms) of death. They will also need to know the name, relationship and address of the next of kin if that is known.
This office will assign someone to assist the family with reporting the retiree's death to the appropriate agencies, stopping the retired pay, filing claims with the finance center for any pay due the family, establishing the Survivior Benefit Plan annuity (if applicable), working with the VA to help the family collect any monies due them from the VA, and assisting the family in having new identification cards issued to them. (note that every family will not be entitled to every benefit). Documents which the family normally need to provide to the casualty assistance officer are: DD Form 214, Retirement orders, marriage certificate, divorce decree and death certificates of the member and/or spouse.
